Transition Assistance Services (TAS)

Transition Assistance Services (TAS) provides financial assistance to individuals with disabilities who are Medicaid-eligible to help set up a household in the community upon discharge from a nursing facility. Individuals who receive services through the Medicaid Managed Care program are eligible, as well as those in Medicaid Waiver programs of Community Living Assistance and Support Services (CLASS), Deaf Blind with Multiple Disabilities (DBMD), and the Medically Dependent Children Program (MDCP). Payment assistance can be provided for security deposits for renting an apartment, establishing utility services, purchasing essential furnishings for the home, moving expenses, pest eradication, allergen control, or a one-time cleaning before occupancy.
